Firefox in LockDown mode with R-Kiosk

Hello,
We are attempting to use Firefox in lockdown mode and running the following add-ons: R-Kiosk and Virtual Hot Keyboard.
When on lockdown mode, Firefox still shows the yellow "Firefox" icon at the top which allows the user to exit and press other options.  When not on lock down mode, this does not show up at all.  
This issue is similar to one that was reported to Inteset on Sept 26, 2011, but it appears it was not addressed.
Hope you can help us resolve it.

By the way, we start up Firefox from the master application with a URL parameter.
eg. http://lab.hakim.se/reveal-js/#/

After a bit more play with it, we were able to turn off the "FireFox" menu icon from the top left while on secure lockdown mode.  

From that menu-> option and click on the menu bar.
That action hid the menu icon.
