Hotkeys not bring up the unlock

I have a machine that was working fine, then after a reboot the the software quit starting the app specified and the hot keys are no longer bringing the unlock screen.  I do not have a second admin account is there a way I can unlock the computer?

I tried this and the steps are cut off when adding the new action.  Could you please post the complete steps?

It's not cut off. That is the last step.
